The foundation for health promotion is a public benefit organization, registered by Regional Court for Kraków -Śródmieście (XI Economic department) and entered into National Court Register under the number 0000210386. Therefore there is an opportunity to donate 1,5 % personal income tax to foundation.

In the year 2022 occured changes in the system of donation of 1,5 % to the public benefit organizations.

To remit 1,5 % of due income tax, it is required to indicate the Foundation for health promotion im. Wiesława A. Smulskiego with the KRS number 0000210386 in the annual statements (PIT) of 2022. The amount equal to 1,5 % of the due income tax will be remitted by the Tax Office to the Foundation for health promotion. In the statement it is allowed to indicate only one public benefit organization.
